01
Getting started
Access, appointments and how ComfortNotes supports your work.
Where are patient information and clinic guides?
Use the footer links for Help & Guides, For patients, system requirements, accessibility, the trust hub and a clinic evaluation checklist. The patient explainer can be printed and complements your clinic’s own notice. These resources do not certify clinical safety or compliance.
What is Ask the evidence?
Open Your workspace → Ask the evidence to visit the dedicated research page for conversational research, reference and education in your assigned Audiology or Physiotherapy workspace. Ask about guidance, studies, treatment evidence or comparisons, then ask follow-up questions. It searches public web sources, not just fixed packs. Other professions are not enabled yet. Answers include source links and an automated source-audit step, but may still contain errors or omissions: check the originals and applicability. It is not an individual diagnosis, prescription, emergency service or clinical approval.
Does research chat use my records or document allowance?
No clinical records or transcripts are read or changed. Enter general questions only, with no patient identifiers or confidential information. Your question and the last three completed exchanges go to OpenAI; search queries may reach its search providers. Chat is not saved in ComfortNotes history and clears when you close the panel, change profession, sign out or refresh; provider retention may still apply. A shared limit of 20 research/experimental requests per account per hour applies, including failed attempts. An accepted research request includes an initial search and a source-audit pass. No consultation credit or document revision is consumed.
What are the three appointment stages?
After selecting your appointment, use Capture for notes and transcription, Review to check, correct and approve the clinical record, and Appointment pack for supporting documents and downloads. Manual document edits are free; AI preparation and revisions use the existing allowance. Moving between stages does not itself generate a document. Each document still needs review and approval before export. Available documents and release restrictions depend on your assigned workspace.
How do I reset my password?
Open Sign in → Forgot password? and enter your account email. Open the newest recovery email in the same browser within 15 minutes. Choose and confirm a strong, unique password of at least 12 characters, then sign in again. Check your junk folder if the email does not arrive. If you use this account for Comforts Hearing too, its password changes there as well; clinic access does not change. Never share a password or recovery link with support.
Who operates ComfortNotes?
ComfortNotes is operated by Homecare Comforts Ltd, trading as Comforts Hearing. It is a distinct product, not a separate legal company. For product support or privacy enquiries, email info@comforts-hearing.co.uk. Do not email patient records.
How does the seven-day clinic trial work?
Trial access is activated separately after clinic approval; registration alone does not start it. No card is required and there is no automatic charge or paid conversion. The clinic shares ten consultations across two staff, including the administrator, for seven elapsed days. Your workspace shows the exact end time. Contact support about trial availability. To continue afterwards, your clinic administrator can choose a paid plan in Plan & usage.
What counts towards my trial allowance?
The first AI-processing request reserves one consultation. Additional recordings and documents within that same saved consultation do not use another consultation credit. Each consultation has 30 minutes of combined recording allowance; live connection and finalisation time count. Initial draft plus two AI revisions are available separately for each AI-generated document; identical requests reuse saved results. Manual editing does not use an AI allowance. Audiogram extraction has a separate three-request limit. Pending work holds allowance until settled; contact support if it remains stuck.
What happens when my trial ends?
New AI processing stops. Authorised access to saved records, manual editing and downloads remains available, with the usual review and approval checks. If all ten consultation slots are used before expiry, existing consultations can continue within their remaining limits. Your clinic administrator can choose a paid plan in Plan & usage. Paid access begins once payment is confirmed and covers the clinic within the selected plan’s limits. Staff should contact their clinic administrator. No payment is taken automatically when the trial ends.
Who can use ComfortNotes?
ComfortNotes supports authorised professionals in approved clinics, within the scope and restrictions of their assigned workspaces. Each approved clinic has separate membership and records. Clinic applications may be open or closed by the platform owner; the registration page shows availability. Invited staff join using an invitation code tied to their email. Use Sign in for an existing account, Join your clinic to accept an invitation or apply for a new clinic, or contact the team about access.
How do I join a clinic with an invitation code?
Ask your clinic administrator for your code. Sign in with the exact email address they invited, open Join your clinic, paste the code and select Accept invitation. You do not need to type the code manually or apply for a new clinic. If you have no account, expand New to ComfortNotes? Create an account, supply the invited email and code, confirm your email, then sign in and accept. Older invitation links still work.
I confirmed my email. Why can I not open the workspace yet?
Confirming your email verifies your account; it does not grant clinic membership. Return to ComfortNotes and sign in with the account you just created. Then paste your invitation code under Join your clinic and select Accept invitation. Do not register again. New clinic applicants instead wait for platform-owner approval. If you cannot sign in, use Forgot password?; if you can sign in but cannot join, check the invited email and ask your administrator to check the invitation.
How does an administrator invite a colleague?
Open Clinic administration, enter the colleague’s email under Invite a colleague, choose their clinic role and select Create invitation. Select Copy invitation code and share it privately with that colleague. The code is shown only when created; keep it until shared. The app does not email invitation codes automatically. Account-confirmation and password-recovery emails are separate. If automatic copying is blocked, the code is selected for you: use Command+C on Mac or Ctrl+C on Windows, then paste it.
What if my invitation code is lost, expired or rejected?
Codes expire after seven days, can be used once and must match your verified sign-in email. Copy the complete code without changing it. If it is lost or expired, ask your clinic administrator to revoke the old pending invitation and create a replacement. A revoked code will no longer work. If you already accepted it, sign in and open your workspace rather than reusing it. An account already belonging to a clinic cannot use an invitation to switch clinics. Never put invitation codes, passwords or recovery links in a support enquiry.
How do I register a new clinic and add its letterhead?
When public applications are open, create an account, confirm your email and sign in. Expand Register a new clinic and enter your clinic name, your name, clinic address and patient-facing contact details. Submit the application for platform-owner review. Staff joining an existing clinic should use an invitation instead. After approval, a clinic administrator can update Clinic details for letters in Clinic administration. New letters use the saved details; previously saved documents do not change automatically.
Will an invitation give access to another clinic or its records?
No. The invitation grants only its specified role in the inviting clinic’s ComfortNotes workspace. Other clinics remain separate. Clinic administration is for authorised administrators; ordinary clinicians do not receive those controls. Consultation notes and photos remain private to their author within the clinic by default. Where clinic sharing is enabled, an author can separately share a selected approved document version with a named colleague. An invitation does not grant access to the Comforts Hearing website.
Can I use real patient information?
Only in a workspace released for clinical use, under your clinic’s patient-data arrangements. An assignment alone does not make a restricted workspace suitable for patient care. If your workspace is marked for testing or fictional information only, do not enter real patient information or record patient consultations. Explain the processing, complete the appointment checks and obtain agreement before recording. Check every transcript and draft. Experimental evidence tools remain restricted to fictional examples; patient photographs are not sent to AI.
How are patient references assigned?
Choose New patient for a new clinic-directory entry. The backend assigns the first three A–Z letters of your clinic name plus a number, such as COM-000001. Returning patients keep that reference: use Find existing patient and check the name and reference against your clinic record. Names alone may be duplicated. Choose Follow-up to find the patient and review your earlier saved appointments, or open Previous appointments beside a linked patient. History is read-only. Continue your current appointment, or start a new follow-up with today’s date, empty notes and fresh processing checks. Earlier findings, photos and approvals are not copied. Directory names and references are shared only with approved staff in your clinic; consultation notes and photos remain private by default; explicitly shared approved document versions are separate. Earlier manual references are preserved in Saved consultations, not automatically imported or merged.
Can I include more than one service?
Yes. Choose a service circle, then use Add or change services for a combined appointment. Microsuction, hearing test, tinnitus, hearing-aid fitting, follow-up and Other are available. Shared notes stay together; the draft is scoped to your selected services. Specialist documentation checks currently cover microsuction, hearing assessment and tinnitus; the other services use general drafting.
02
Profession workspaces
Your assigned tools, within one clinic account.
Why is the homepage focused on audiology?
ComfortNotes began with audiology and the aim of helping clinicians spend less time organising documentation and more time focused on the person in front of them. Other professions face similar documentation demands, which prompted the development of additional profession-specific workspaces. Audiology remains on the homepage to reflect where ComfortNotes began; eligible users open their additional assigned workspaces through Account. More profession-specific workspaces are planned, with availability expanding as templates, functionality and clinical review are completed. Read why ComfortNotes is expanding.
Who assigns my profession workspace?
Your clinic administrator assigns the workspaces available to your clinic and eligible for your account. An administrator cannot enable an unreleased workspace or override a restricted test. Contact your administrator if you need an assignment checked.
Where do I find my assigned workspaces?
Sign in and open Account to see your assigned workspaces. Where an Open button is shown, select it to enter that workspace. Not every workspace is available to every clinic or account; the available options depend on your clinic’s access and your assignment. Follow any scope, availability notices or testing restrictions shown inside the workspace.
Does every workspace use the same documents?
The overall process is similar: type or dictate your notes, check the transcript, review and approve the clinical record, then prepare and approve the supporting documents available in that workspace. Templates, document options and specialist tools vary by profession. Do not assume that a tool or letter available in one workspace is available in another. Nothing is sent automatically.
Do additional workspaces give us a separate subscription or allowance?
No. Assigned workspaces use the same clinic subscription or trial and its shared consultation allowance, subject to the current plan limits. A staff member with more than one assigned workspace still counts as one staff account. Additional workspace access does not add consultation credits, recording time or document-generation allowance.
Can I move a consultation to another profession workspace?
No. Each saved consultation stays with its original profession. Open the appropriate assigned workspace when starting a new consultation; switching workspaces does not convert an existing record or its documents. Saved records keep their existing access controls.
Does a workspace assignment let me read colleagues’ consultation notes?
No. Workspace assignment gives access to the relevant tools, not to colleagues’ consultation notes. Notes remain private to their author within the clinic, and other clinics cannot access them. The clinic patient directory is shared with authorised clinic staff; this does not grant access to colleagues’ clinical records. If clinic sharing is enabled, an author may explicitly share a selected approved document version with a named colleague, including someone in a different assigned profession. It is a read-only copy, not access to the whole consultation. Later corrections are not shared automatically.
03
Notes & recording
Your words first. A reviewed draft next.
Do my notes need a special format?
No. Write short notes in your own words: what brought the person in, what you found or did, and what was agreed. The optional Dictation prompts stay above the notes while you type or record and can be collapsed. Expand Clinical sections to cover for the actual headings used by your workspace. A clear point for each relevant heading helps the draft capture the information. This does not guarantee completeness: check the result and leave unknowns unknown. Use any order; these headings are not inserted into notes or sent as patient evidence. Include only known facts; no extra form is required. Hearing-test measurements are optional under Add test results. Review the clinical draft to clarify anything missing. Service guides and read-only fictional examples are in Help.
What happens when I select Prepare clinical record?
The AI processes your source notes, any transcript and relevant confirmed written photo findings to fill the clinical record sections. Review and approve that record, then select Prepare appointment pack for letters and follow-up documents based on its approved wording. It aims to organise the findings and use wording appropriate to the assigned profession without changing the original notes. Details to check highlights gaps or possible conflicts; it is not an exhaustive safety or accuracy check. AI can omit or invent information, so compare every draft with the source and your own findings.
In audiology, do I need to repeat my notes under Clinician assessment?
No. In the audiology workspace, for every appointment type, including combined appointments, findings, tests, care and outcomes stay in their appropriate sections. This covers microsuction, hearing tests, tinnitus, hearing-aid fittings, follow-ups and other consultations. An additional clinical conclusion is optional: record it if you have something further to say, including uncertainty or limitations. A separate empty assessment is not counted as a missing detail. Any conclusion you do record remains part of the record and letters; conflicting information still needs clarification. Checks for missing service-specific information remain, including actual test results and tinnitus assessments or scores. The app never invents a conclusion or treats missing information as a normal finding. Other professions use their own recorded assessment and reasoning sections; follow the review prompts in your assigned workspace.
How can I dictate my notes?
Yes. Choose Dictate notes or Live dictation from the recording mode menu beside the record button, then press the button to start. Sign in, complete the information and processing checks, obtain everyone’s agreement and allow microphone access. You can pause, resume and stop. Live mode shows text as you speak. AssemblyAI provides plain-text transcription without speaker labels. Best suited to one clinician dictating; make clear when describing what a patient said. Audio clips are sent for transcription after each 2½-minute segment or Stop in Dictate notes mode. Both modes process audio before you select Prepare clinical record. Complete the pop-up and select Confirm & start recording. Your typed notes are kept; audio playback and retry are under Recording details. Always check the transcript.
What is the audiology Individual Management Plan?
In the audiology workspace, select the optional Individual Management Plan checkbox beside Patient letter. After approving the clinical record, prepare the appointment pack to organise its recorded needs, goals, planned and completed actions, responsibilities, outcomes and next review. The plan copies relevant approved clinical sections, so a section covering several topics may appear more than once. Missing details stay Not recorded; review the grouping and wording, then approve and export. The template is informed by BSA adult rehabilitation guidance (October 2016, section 4.2), not approved or endorsed by BSA. At follow-up, read previous plans in Previous appointments and record today’s agreed changes in today’s clinical note. Earlier plans and approvals remain unchanged; there is no automatic carry-forward.
Can I upload an audiogram?
Yes. Under Hearing test → Add findings or test results, open Audiogram. Upload a PDF, JPEG or PNG up to 4 MB and explicitly authorise OpenAI extraction, or enter measurements manually. Check the patient, every threshold, ear, frequency, masking and no-response marking before confirming. Only confirmed results join your typed notes and transcripts when preparing the clinical record. This does not diagnose or reconstruct a graph. Original files remain in this tab only: keep them in your clinic record. Entered measurements save with the consultation; editing them requires renewed confirmation.
Which documents will I receive?
Document options vary by workspace. In audiology, the clinical record comes first. Approve it, then prepare the remaining pack: a personalised patient letter in plain language with thanks and Yours sincerely; your selected Letters & referrals (GP update, ENT referral, another clinician or service, or a custom letter) with a separate recipient and purpose for each; a hearing report if the approved record contains numeric hearing results; an optional Individual Management Plan; and follow-up tasks copied from its agreed plan. Missing results are never reconstructed. Correcting the clinical record does not silently change letters: re-approve it and review the affected documents. Where available, Compare & confirm document lets you confirm manually updated wording without an AI revision; Update appointment pack with AI prepares proposed replacements. Saved versions remain in history.
What can a custom letter do?
Use Letters & referrals → Custom letter for a one-off letter about the appointment, for example to share the recorded care with a named professional or respond to a request for an appointment summary. Enter the recipient and what you want to communicate. It uses only the approved clinical record, with your clinic letterhead, a greeting and Yours sincerely. The purpose is not a substitute for missing findings. It does not create insurance, occupational-health or medico-legal opinions. You must check the recipient, content and permission to share, then approve and transfer it through your clinic’s secure process. Nothing is sent automatically.
Do I have to approve the AI wording?
You do not have to accept it as written. Check the source and correct the draft. The approval button opens the review panel if review is outstanding. Confirm source accuracy, patient details, ear side, measurements, partial sections and flagged points. Explain any details left unrecorded; do not invent missing care. Resolve flagged conflicts before approval. In audiology, confirm the full corrected section through Add clarification or Confirm section correction; this does not require AI regeneration. Then approve and save. Supporting documents remain optional and need separate approval. Clinical edits require renewed review; changing original source information can require fresh drafts. The dated standards register explains the references and remaining mapping limitations. This is documentation support, not diagnosis or compliance certification.
Which standards inform the records and letters?
Each supported profession has its own reference guide and review prompts; use the guide in your assigned workspace. In audiology, the guide links selected HCPC conduct standards and hearing aid dispenser proficiency standards, relevant BSA documentation guidance and NICE adult hearing-loss and tinnitus recommendations. Each letter has a short Before approving guide. Open Details to check → Standards & review dates for the editions and mapped sections. These are ComfortNotes templates informed by guidance, not BSA- or HCPC-approved templates. The app checks source references and required recorded sections; the clinician checks clinical meaning, disclosure and any local referral requirements. Guidance never fills in missing care, decides urgency or sends a referral. Current reference updates do not rewrite older approvals.
04
Reviewing & correcting documents
Get the clinical record right, then prepare the documents you need.
What if the clinical record is already correct?
Check it against your typed notes and completed transcripts, resolve any review prompts, then approve and save. You do not need to add a correction or generate it again. Prepare the appointment pack from that approved record, then review and approve each supporting document you intend to use. Nothing is sent automatically.
Why does a section still say Not recorded after I edit its wording?
The displayed document and its source-review status are separate. In audiology, open Details to check and use Confirm section correction or Add clarification. Enter the complete final wording for that section, choose an accurate status and confirm it. This records a clinician-authored correction, retains the original source and requires renewed approval; it does not claim the original transcript contained the addition. Do not record an unknown finding as normal.
Will corrected clinical notes automatically update my letters?
No. First save, review and re-approve the clinical record. Choose Update appointment pack with AI, check the documents and remaining allowances, then compare and accept the replacements you need. Review and approve each updated document. Manual editing remains available without AI: use Compare & confirm document in audiology or Update this document manually in another enabled workspace when only the clinical record changed. Original source changes still require separate review. Previously downloaded or shared copies do not change automatically.
Do manual corrections use my two AI revisions?
No. Manual wording edits, section corrections and approval do not use an AI revision. The initial draft and two AI revisions apply separately to each AI-generated document. Preparing AI replacement wording or an AI pack comparison can use that document’s allowance, even if you keep the old wording. Identical requests reuse saved results. There is no lifetime manual-edit count. Current audiology section amendments allow up to 2,000 characters each; replaced amendments remain in saved versions, and evidence still cited elsewhere is retained. Text and overall record-size limits still apply. If a limit or save error appears, keep your work and contact support rather than repeatedly generating.
What if earlier saved review checks are missing?
In audiology, Restore saved review checks can recover compatible checks from saved history when an earlier clarification cleared them. You must confirm the complete wording of each affected section, then review and approve again. Recovery is not available when other source information changed or no compatible saved review exists. Keep your source notes and contact support if recovery is unavailable; repeatedly generating is not a repair for missing saved checks.
What is the difference between editing source notes and correcting the document?
Source notes and transcripts are the material used to prepare the first draft. Changing them after generation can invalidate the draft and require fresh generation. To correct a generated document, use its wording or section-correction controls where available, then complete renewed review. Original source information stays distinct from clinician-authored corrections. Always check the whole record: a missing-information prompt is not a complete clinical safety check.
05
Otoscopy photos
Clear labels, with your observations in control.
What is the Otoscopy documentation assistant?
It helps you document your own examination. Open Otoscopy photos, upload and check the left/right and stage labels, enter your observations and limitations, and confirm them. The existing notes AI uses your confirmed writing when you select Prepare clinical record; it does not inspect the photograph or add image-derived findings. Check and approve the record before preparing the remaining appointment pack.
What is in the otoscopy reference library?
Open Otoscopy reference library · guidance & terminology beside your photographs or in Learning & Resources. It contains BSA, HCPC and NICE publisher links, basic terminology and a fictional documentation example. Publication information, an editorial review date and reuse limitations are shown. This reference-link section does not import articles or photographs. The separate teaching gallery includes licensed photographs with credits and review status. These reference links are not an approved AI evidence collection or an endorsement of ComfortNotes.
Does the reference library suggest a diagnosis?
No. You browse the references yourself. It does not compare a patient image with reference images, suggest conditions or treatment, or insert example wording into a patient record. Its content is not sent to AI. Use your professional judgement and current guidance; record only your own findings.
Are my patient photographs used for AI training?
No. Patient otoscopy photographs are not sent to AI for interpretation or used for training, fine-tuning or a shared reference collection. Only confirmed clinician-written observations can enter the notes-drafting workflow.
Does the app label left, right and the image stage for me?
Yes—from the labelled box you choose, not from recognising the image. Select a left or right before/after box for microsuction, or a left or right assessment box for a hearing-test or tinnitus visit, then choose a file. The photo uploads and receives a readable label automatically. Check it afterwards; Correct ear or stage lets you fix a mistake. Changing a label clears its confirmation and marks existing drafts outdated. Clinician review & sign-off keeps consent, consulted links and clinician decisions separate for each photo. Correcting its ear or stage resets that review’s consent and references.
Can the AI detect an infection or diagnose from a photo?
No. Automated image evaluation is unavailable. Uploading, reviewing or confirming a photograph never sends it to AI. Open Clinician review & sign-off to record the choice for that photo, assess the view and enter your own findings, limitations and conclusion. Confirm your observations only after checking the original. Only your confirmed writing can support the notes AI. Optional research links are for you to consult; they are not automatic image comparisons. Changing a label or finding clears confirmation.
Which images can I add, and can I compare them?
Use exported JPEG or PNG otoscopy photographs, up to 8 photos per consultation, each no larger than 5 MB or 16 megapixels. This is a file-upload workflow, not a direct otoscope connection. Original files and embedded metadata are preserved. You can enlarge a photo and compare before-and-after photos of the same ear.
What happens if I remove a photo?
Remove from consultation detaches the photo from the current consultation; it does not delete the private original or earlier saved versions. Existing wording is kept but marked outdated, and approvals are removed. Use Retention & holds to record an applicable review date or a hold. Automatic deletion is off; a disposal-review request does not delete anything. Do not treat removal as permanent deletion.
06
Saving & sharing
What stays, what exports and what happens next.
Does ComfortNotes follow NHS England’s AI-scribing guidance?
ComfortNotes is being developed with reference to NHS England’s guidance on AI-enabled ambient scribing. Its workflow supports clinician review, correction and approval of AI-generated documentation. Formal clinical-safety, information-governance and deployment assurance remain separate requirements. This is not a claim of NHS approval, certification or full compliance with the guidance. Organisation-specific assessment and authorisation are still required before NHS deployment.
How do I chat with ComfortNotes support?
On the homepage, open Need help? and type a product question for an answer from our help library, without AI. If the question is unclear, choose a related answer or use Contact support. Signed-in users who have completed any required two-step check can select Contact support → Chat. Messages and replies are saved separately from clinical records and refresh every five seconds while the chat is open and visible. Only you and authorised platform-support administrators can access your conversation—not other clinics or your colleagues. Available means a support inbox is actively connected; response times are not guaranteed. When away, leave a message, email info@comforts-hearing.co.uk or call 07916 708703. Chat has no attachments or AI replies. Do not send patient information, passwords or clinical questions. Close chat to stop updates; reopen it to see replies. There are no email or push notifications for chat replies. Routine support records are reviewed 12 months after closure; automatic deletion is not enabled.
Where can I read about security and UK GDPR?
Read Security & Data Protection for storage, connections, access controls and provider security links. Our Privacy Policy explains processing, providers, retention and your information rights. Provider credentials are not certification of ComfortNotes; disclosed supplier and operational limitations still apply.
Are my notes saved automatically?
When you are signed in and the required acknowledgements are complete, consultation changes save automatically. Wait for All changes saved before leaving. If saving fails, select Retry and keep the tab open until saving is confirmed. Use Saved consultations to reopen saved work. Opening help within the app keeps the current consultation in the tab, but refreshing or closing can lose unsaved work. Wait for uploads and processing to finish first.
Where are my notes, photos and recordings kept?
Saved consultations and photo originals are private to your account within your clinic’s ComfortNotes workspace. Other clinics cannot access your records. Notes and audio are processed by the connected AI services when you request drafting or record for transcription. Raw recording clips are held in the current browser tab; saved transcripts are part of the consultation, but clips are not restored when you reopen it. Your clinic must follow its approved privacy, retention and clinical-review arrangements. Read for the processing arrangements.
What can I export, and is anything sent automatically?
Clinical records appear in full-width grouped tables. Details to check shows the outstanding count and opens the review panel; closing it does not resolve any gaps. Use Edit wording to make corrections, then View formatted document to check the presentation. Changes require renewed review and approval. Custom or earlier layouts are shown in full if their sections cannot be recognised safely. Approve and save a document to use Copy to clinic record, download its plain text or use Print / PDF for the formatted record, including saving as a PDF through your browser. Clinical records use an A4 landscape print layout; your browser’s print settings can override this. The document cards make every generated draft accessible immediately. Patient letters, letters and referrals, hearing reports and follow-up tasks use portrait layouts. Approve the clinical record first, then review and approve each additional document you want to export. Photographs are not included in text or PDF exports. Copy places text on your system clipboard; paste only into your approved clinic record system, check the patient reference and formatting, and clear the clipboard when finished. This is manual transfer, not a NOAH or practice-system integration. The app does not automatically email, message or send documents to patients or referrers.
What is Learning & Resources?
Use the header or footer link, or Your workspace → Learning & Resources for forms and questionnaires, ear conditions teaching images, and guidance and research links. Forms include THI, THS, Hearing Goals & Review and original templates. Answers stay in the browser: they are not autosaved, sent to AI or added to patient records. Review and download a PDF, then clear the form; downloaded copies remain on your device. COSI and TFI remain publisher links pending permissions. Teaching photographs have credits, reuse information and clinical-review status; they are not matched to patient images. A separate collapsed experimental tool accepts fictional written examples only and retains your existing evaluation history. Authorised source managers and reviewers have a separate administration area. These tools do not interpret photographs or insert findings into clinical notes.
Where can I find forms and questionnaires?
Open Learning & Resources, then Open forms & questionnaires. Choose a form to complete in a separate browser tab. The library includes THI, THS, Hearing Goals & Review and original templates. COSI and TFI are publisher links only; Hearing Goals & Review is not COSI or a validated replacement.
Are questionnaire scores calculated automatically?
The THI calculates a total out of 100 after all 25 questions are answered; an incomplete form has no total. THS calculates separate tinnitus and hearing section totals when the relevant questions are complete, shown during review and in the PDF; there is no combined score. Other templates do not automatically receive a score. Scores need clinician review and do not provide a diagnosis or treatment recommendation.
How do I download a completed form as a PDF?
Complete the form, open its review step and check the answers. Confirm your review to enable the PDF download. If you edit answers, review them again before downloading. Check that the file is in your Downloads before clearing or closing the form, and store it according to your clinic’s arrangements. Nothing is automatically emailed or added to a clinical record.
Are answers in the forms library saved?
No. Form answers are held only in the current browser page, not saved to your ComfortNotes account or sent to AI. Closing, refreshing or clearing the form can lose them; they cannot be reopened from Saved consultations. Download and check your PDF first. Clearing the form does not delete a PDF already downloaded to your device.
Does the evidence library or AI learn automatically?
No. Source checks are initiated manually in this release, and overdue, changed or unavailable sources are held back. A page check is not a clinical review or a guarantee that research is current. Feedback queues evidence issues for human review; it does not retrain the model. Patient images must not be used for training, fine-tuning, shared reference collections or model-improvement exports. Clinical image interpretation remains disabled. Automated image evaluation is unavailable in this release. The photo workflow is clinician-written only. Source reuse permissions and clinical review are separate checks.
What if I need help or something goes wrong?
Keep the tab open if a save or upload has failed, read the on-screen message and use the available retry control. Avoid refreshing while work is unsaved. For product questions, contact the team and describe the screen and error. Never send patient information, recordings, photographs, passwords or API keys in a support enquiry.
